Caregivers/Guardians are required to log in to their account and complete the following three forms listed below by no later than June 1st. Camp forms submitted after this date will incur a one-time late paperwork fee of $50 per camper.

  • Understanding Our Campers – Adults

  • General Terms, Conditions, Waivers & Release Form

  • Medical Form

    Your camper’s physical, immunization record and doctors orders (medication list) must be issued and signed by your camper’s primary health care provider and submitted no later than June 1st. Medical documents submitted after this date will incur a one time late paperwork fee of $50 per camper.

    Caregivers/Guardians are required to log in to their camp account and upload the medical documents listed below. We have provided templates for your convenience but your doctor’s office can issue their own. PCP’s are welcome to fax documents to the camp office directly (978) 225 2745.

  • Immunizations Record Form Aged 18+*

  • Physical template – Adult Campers*

  • Doctors Orders for Medications (if applicable)* — Must be current and signed by health care provider
