Grotonwood complies with the regulations of the Massachusetts Department of Public Health (105 CMR 430,000), is licensed by the Groton Board of Health and inspected by the Groton Fire Department. For the safety and security of all our campers and staff, we are required to obtain certain medical and pre-camp information by June 1st - late fees apply.
Step One: Pre-Camp Forms
Caregivers for both for both day and overnight campers must complete the three forms listed below as part of the online enrollment process. If you haven’t already, simply log in into your camper's account and click on Forms to get started.

Step Two: KIDS overnight MEDICAL Documents
We require the following three medical documents from your child’s Primary Care provider by June 1st.
Immunizations Record Form Aged 7 to 11 OR Immunizations Record Form Aged 12 to 17
If applicable - Doctor’s Orders for current medications (must be signed by your camper’s primary care provider)
We have provided templates (linked above), however, we prefer doctor’s offices to issue their own. Your child’s primary care provider (PCP) can fax their physical, immunization record and a doctor’s order for their medications directly to the camp office at (978) 225 2745.
These documents must be issued and signed by your camper’s PCP no later than 18 months prior to their arrival at camp. Once signed and completed by your child’s PCP, please upload them to your camper’s account so our clinical team can review them.
STEP TWO: Day Camp medical documents
We require the following medical documents from your child’s Primary Care provider by June 1st.
Immunizations Record Form Day Camper Aged 5 to 11 OR Immunizations Record Form Day Camper Aged 12 to 17*
Health History Form – Day Camp (for day camp, this form can be signed by a parent/guardian)
If applicable, Doctor’s Orders for current medications to be administered at camp including rescue meds like inhalers and epi-pens.
Pre-camp forms and medical documents must be submitted no later than June 1st - late fees apply. For ease, your Child’s PCP can fax medical documents to the camp office directly at (978) 225 2745.
