KIDS medical FORMS

Grotonwood complies with the regulations of the Massachusetts Department of Public Health(105 CMR 430,000), and is licensed by the Groton Board of Health. We are therefore required to obtain certain medical and camp information ahead of arrival.

To complete the forms online, simply login into your camper's account and click on Forms & Documents.

Alternatively, you can download the medical forms by clicking on the links below and upload those to your camper’s account.

Camp forms and medical documents must be submitted no later than June 1.

Paperwork submitted after the deadline will incur a $25 per camper, per week fee.

Day Camp Health & Camp Forms

Required Forms:

• Immunizations Record Form Day Camper Aged 5 to 11 *

• Immunizations Record Form Day Camper Aged 12 to 17*

• Health History Form – Day Camp (can be signed by parent/guardian)

• Medication List (if applicable)*

KIDS overnight CAMP MEDICAL Forms

Required Forms:

• Immunizations Record Form Aged 7 to 11 *

• Immunizations Record Form Aged 12 to 17*

• Physician Examination Form - Youth Overnight*

• Current Medication List signed by your camper’s primary care provider (if applicable)*

  *These documents must be issued and signed by your camper’s primary health care provider.
