YOUTH camp medical FORMS

Grotonwood complies with the regulations of the Massachusetts Department of Public Health(105 CMR 430,000), and is licensed by the Groton Board of Health. We are therefore required to obtain certain medical and camp information ahead of arrival.

To complete the forms online, simply login into your camper's account and click on Forms & Documents.

These forms must be submitted no later than four weeks prior to your campers session start date.

Paperwork submitted late will incur a $75 Late Paperwork fee.

• General Terms, Conditions, Waivers & Release Form

• Over The Counter, Emergency Treatments & Medical Permissions Form

• Health History Form – Day Camp

• Medication List (if applicable)*

• Medical Insurance Form
