Caregivers/Guardians are required to log in to their camp account and complete the three forms listed below:

  • Understanding Our Campers – Adults

  • General Terms, Conditions, Waivers & Release Form

  • Medical Form

    UPLOAD Required DOCUMENTS BY CLICKING HERE:

    Caregivers/Guardians are required to log in to their camp account and upload the medical documents listed below. We have provided templates for your convenience:

  • Medical Insurance Form for Adult Campers

  • Immunizations Record Form Aged 18+*

  • Physical template – Adult Campers*

  • Medication List (if applicable)* — Must be signed by health care provider

    *These documents must be issued and signed by your camper’s primary health care provider.

    These forms must be submitted no later than June 1st.

    Paperwork submitted after this date will incur a late paperwork fee of $25 per week.
